How It Works

Choose your language and your format. The PDF can be typed into directly — click any field and write, and click the small squares to tick them.

Take your time
Answer as fully and as honestly as you can. If a question does not apply, leave it empty. If something feels too tender to write, write in person and we will speak of it together.

Send it back
Save the file with your own name — for example MenoDawn-Questionnaire-Maria.pdf — and email it to me. I will read every word before we meet.
Everything you write is held in complete confidence. Your answers are seen only by Amma Usha Regina, and where necessary by the facilitators supporting your care. They are used solely to prepare and adapt your sessions or your retreat, are never shared or sold, and you may ask for them to be deleted at any time.
